- Have connections and resources to support health at individual and community levels.
- Conduct basic health testing and can help locate and connect you to health care providers.
- Deliver health education to groups and individuals.
- Support people in reaching their health goals.
Public Health Nurse
- Focus on preventing illnesses and increasing health for whole communities.
- Test for a variety of health conditions.
- Help connect you to providers/services and can follow up to make sure everyone understands.
- Supports individuals with their self-care of medical and mental health conditions.
- Deliver education to groups and individuals.
