Community & Home Services
In what is now known as the Olmstead decision, the Supreme Court decided that people with disabilities are best-served alongside everyone else. They must be integrated in the community as much as possible, with the support programs and services necessary to be successful. Ramsey County supports employment, housing and other integrated, person-centered services as required under the Minnesota Olmstead Plan.
The following Minnesota Department of Human Services programs help with “person-centered” services in your community.
Consumer Directed Community Supports
People who receive community waiver services can chose Consumer Directed Community Supports. This option gives you the flexibility to plan the right mix of services and supports for your needs. You will be responsible for hiring and managing your direct care staff. Contact your social worker or call 651-266-3613 for more information.
Consumer Support Grant
The Consumer Support Grant (CSG) gives individuals choices and alternatives to state-funded Medicaid home care services including home care nursing, home health aides, and personal care assistance. Participants must be eligible for Medical Assistance and may use CSG funds to purchase a variety of goods, supports and services beyond traditional home care.
Approved goods, services and supports include, but are not limited to:
- Adaptive equipment.
- Chore services.
- Companion services.
- Family counseling.
- Home adaptations.
- Home delivered meals.
- Personal care assistant services.
- Temporary care to provide relief for caregivers.

Employment and vocational services
Working is the key to living in the community. Ramsey County believes that people with disabilities have the same rights and abilities as people without disabilities, including the right to work at regular jobs. A case manager/social worker will work with you to find supports that will assist you in finding the right job in the right setting, while maintaining your benefits. They will focus on your strengths as well as your skills or interests that can lead to jobs.
