Jan. 16 annual report open house
Get technical assistance from inspectors as you fill out your 2024 hazardous waste annual report.

- No registration needed.
Prep for the open house
- Bring a laptop or other electronic device. WiFi and extra laptops are available.
- Bring documentation, like manifests or disposal receipts, to help you fill out your report.
Annual report emails will be sent in early January and are due January 31, 2025.

Record keeping forms
Hazardous waste generators are required to keep documentation of certain activities. Learn more
For all generators
Generators are required to inspect and log inspections of their hazardous waste storage areas and containers.
Emergency planning
Small Quantity Generators (SQG)
Small Quantity Generators are required to make arrangements with local authorities in the event of a release. Use the form below to notify local authorities. Small Quantity Generators are also required to have a telephone posting. Complete page five of the local authority notification form, and post it near any phone used during an emergency.
Large Quantity Generators (LQG)
Large Quantity Generators are required to complete a contingency plan.
Training form
Train all employees that have hazardous waste management and emergency response duties. This form can be used to track required training.
