Minnesota Health Care Programs can give you and your family coverage for most medical services or provide help paying your Medicare premiums, deductibles and copays. How much help you can get depends on the program you qualify for.

Health care application for certain populations
If you are: 65 years old or older; blind or have a disability; only requesting help with Medicare costs; receiving Supplemental Security Income; or are applying for Medical Assistance for Employed Persons with Disabilities, you should apply for health care assistance using the Health Care Application for Certain Populations.

Long-term care services
Anyone with a disability in need support services to stay in their homes should take a free MnCHOICES assessment. MnCHOICES helps individuals make decisions about long-term services to help thrive in their communities. Schedule a free MnCHOICES assessment by calling 651-266-3613.
Ramsey County case management
Ramsey County’s Minnesota Eligibility Technology System (METS) team manages cases in the MNsure system for Ramsey County residents who receive Medical Assistance. If you have a Medical Assistance case in the MNsure system assigned to Ramsey County:
- Contact the METS team with questions Monday-Friday, 8 a.m. – 3:30 p.m. at 651-266-3870.
- Fax applications or documentation to the METS team: 651-266-3934.
Health care assistance available in Minnesota
Ramsey County staff can help residents access and navigate most healthcare programs.
- Medical Assistance: Minnesota’s Medicaid program and largest publicly funded health care program. Ramsey County can help you determine if you are eligible for Medical Assistance.
- MinnesotaCare: A health care program for Minnesotans with low incomes.
- Minnesota Family Planning Program: Covers only family planning services for people not already on Medical Assistance.
- Home and community-based waiver programs: Assistance for people who need extra help to stay at home instead of moving to a nursing home or other medical facility.
- Medicare savings programs: A savings program for people who have low income to help pay the cost of their Medicare coverage.
- TEFRA insurance: Insurance for children with disabilities whose parents do not qualify for other state-funded health care programs due to their income or who qualify for those programs but the cost would be too high.
Managed health care options
If you qualify for a Medical Assistance (MA), you may need to choose a health plan to receive health care services. To find the health plan that fits your needs, visit the Minnesota Department of Human Services’ Health Plan Selection Resources page.
Transportation to medical appointments
Minnesota Non-Emergency Transportation (MNET) provides transportation to medical appointments for people on Medical Assistance, some pregnant women, and children on MinnesotaCare who have no other transportation available.
If you are enrolled in a managed health care plan, call your plan for transportation assistance.

Resources and reimbursement
If you need reimbursement for mileage and parking expenses even if you are in a health plan, or if you receive your care "fee for service" and need assistance with medical transportation:

Your rides can be booked online through MTM Link.
Allowable medical transportation expenses
- Bus fare.
- Taxi fare.
- Light rail transit fare.
- Mileage reimbursement (call in advance).
- Parking with a receipt.
- Trips outside the local area, including transportation, lodging, and meals (call in advance).
